  • Appropriate Loss, Premium, Payroll Histories and with Employer FEIN#
  • Broad description of operations
  • Experience modification worksheet
  • Average wage level for major class by state
  • ERM14 (when needed)
  • Completed selection worksheet (nine questions on one page)
  • Applicable experience rating data
  • Copies of most recent Federal Payroll Tax Filing (941) to support Payroll estimate
